Waiver of Liability

I hereby understand and acknowledge that the training, programs and events held by the trustees of the JLS Family Trust, its trustors, beneficiaries, partners and affiliates (collectively “JLS”) may expose me to many inherent risks, including accidents, injury, illness, or even death. I assume all risk of injuries associated with participation including, but not limited to, falls (while in the building or on the premises including adjacent sidewalks), contact with other participants, contact and interaction with equipment, incidental contact by teachers and staff, the sudden and unforeseen malfunctioning of any equipment, the effects of the weather (including high heat and/or humidity), firearm discharge, and all other such risks being known and appreciated by me.

I hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity. I acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in.

To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of the facility, its owners, members, donors, agents, and employees. If any portion of this release from liability shall be deemed by a court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect, and the offending provision severed from the form. This Waiver of Liability may not be modified orally.

After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and JLS furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE JLS, its officers, agents, employees, organizers, representatives, affiliates, parent entities, donors, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in the JLS training, programs, events or any combination thereof.

By my signature, I indicate that I have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms. This Waiver of Liability has no expiration and will be valid for all JLS training, programs and/or events in which I participate, including repeat participation.
